 Social work Hub provides specialist complex care for children & adults with life-limiting and long-term conditions.  Working in partnership with Clinical organizations, Social Services and Case Managers, our services enable those with severe disabilities to receive the care and support they need in the familiar surroundings of their own homes.

Care is usually delivered by Registered Social Workers, Nurses or by Health Care Assistants through a clinical competency-based training programme.  We are equipped to meet the needs of children & adults with a range of complex health conditions and care requirements, including:
